Not at all sure this is one of the family, but looks like the death of Jacob Conrad on 5 June 1731, at age 5 months and 4 days. Parents Johann and Magdalena?

On June 5, 1731 died
the wife of Conradt Matthes - Magdalena
buried the following day
63 years 5 months and 4 days old.
