Does this imply the early delivery was an additional cause of the mother's death, or the pleurisy caused the early delivery?

I suppose both causes of death stand parallel.
Otherwise it should had been stated somewhat like:
"Zu frühe Niederkunft infolge Brustfellentzündung"
But I am not a doctor, sorry.

You have to take into consideration, that the diagnosis back then was not as detailed as a modern day doctor in 2022 would state.
Let alone a pastor who has no medical education but only notes what the doctor told him.
